Patient shows flashes of extreme anger. Patient refuses medication. Patient refused analysis. Patient attacked 2 staff nurses & room-mate. Patient attempted escape several times. Patient attempted suicide on three occasions. Patient attempted murder on 1 occasion.

Update:17th November 1992
Patient has shown much improvement in the last few months. Slight paranoia still exists although most likely due to patient using cannabis.
Will recommend to the board for release.
